Coaches,

Open the attachment to see the announcement we've created for Physicals Night, Wed, June 4. Please share this information with your student-athletes and parents via email, phone calls, banquets, or team/parent meetings ASAP. Please note, there are two (2) required forms we need to have completed and available to the athletic department PRIOR to ANY PARTICIPATION in practices starting this July.

The first is the Medical/Travel Release form the parents complete on line. This form has three (3) sections; Emergency Medical Authorization, Health History, and Over-The-Counter Medication Authorization. Parents can accompany their children (grades 7 - 12) to physicals night June 4 and complete/review the Medical/Travel Release form in the computer lab in Dennett Hall at the same time the student-athletes are receiving their physicals.

The pre-participation physical exam (PPE) form is the second document we need on file. Parents fill out pages 1, 3 and 4. The evaluating physician completes page 2. All PPE forms are to be turned in to the Athletic Department once completed and signed by a parent, if the student-athlete is not 18 years old. No activity at least 1 hour prior to having the physicals done.

Middle School 6:45 pm

Parents and students meet in the Dining Hall.
(Screening exams are held in Dennett Hall after sign-in at the Dining Hall)

This opportunity is offered to all student-athletes enrolled at CA entering grades 7 thru 12 in the 2008 – 2009 academic year. All student-athletes must have BOTH their on-line Medical/Travel Release form & physical form (PPE) completed and on file at Columbus Academy PRIOR to attending the first practice. The Medical/Travel Release form has 3 sections for you to complete or review; Emergency Medical Authorization, Health History, & Over-the-Counter Medication Authorization. The PPE form is valid for one calendar year from the exam date. Blank forms are available on the CA intranet (Athletic Health Forms), on the OHSAA web site, in the Athletic Department, in the Athletic Training Room, and in the Dining Hall the evening of the exams. Turn in all completed PPE forms to the Athletic Department.

Page 1 (along with pages 3 & 4) of the PPE form needs to be completed and signed by a parent/guardian PRIOR to the exam, giving the physician permission to evaluate the student-athlete. Please note that this is only a sports screening exam. It does not take the place of a thorough well-child check that is performed by your primary care physician.
